The Flu Vaccine Documentation Submission Survey is now closed for the 2021 mandatory vaccine program. You can no longer submit your non-MGB FLU vaccine documentation through this portal.

Mass General Brigham Employees and POIs who need to submit vaccine documentation to meet compliance OR wish to submit COVID-19 Booster Documentation received at a non-MGB institution, should email proof of vaccination to Occupational Health System Services as follows:

Flu Vaccine documentation to occhealthfluvaccination@partners.org
COVID Vaccine and booster documentation to PHSOHSCOVID19@partners.org

Employees who received vaccine at an MGB site and need to consent to share their vaccine record from EPIC, can enter in their consent through Peoplesoft.

If you are a New Hire, please submit your COVID-19 and Flu Vaccine Documentation to PHSOHSNewHire@partners.org.
