Welcome to the intake form for the Eating Disorders Clinical & Research Program at Massachusetts General Hospital (MGH). This form is for prospective patients (and parents/guardians of minors) only. If you are a provider, please refer your patient to fill out this form on their behalf.
We are an outpatient eating disorders program located in Boston, MA. We have several clinical psychologists and two psychiatrists on staff. We are able to provide time-limited evidence-based treatment for eating disorders, typically cognitive-behavioral therapy for adults and family-based therapy for children and adolescents.
Please note that, in order to coordinate medical and mental health care, we are only able to accept patients with an established relationship with a primary care or Adolescent Medicine physician at MGH.
Please note that PCPs must be located at MGH (not at any Mass General Brigham-affiliated facilities, like Brigham and Women's Hospital, Partners Healthcare facilities, etc.). Additionally, other types of providers (e.g., GI providers, neurologists, psychiatrists) are not able to meet this requirement.
Providers who meet the Adolescent Medicine requirement include: Drs. Mark Goldstien, Katie Brigham, Nupur Gupta, Karen Sadler, Jennifer Rosenblum, Mariette Murphy, and Diana Lemly.
If you (or your child) have an MGH PCP and are interested in pursuing care at our clinic, please fill out this form. Please note, we are currently booking out ≥6 months for evaluations. However, we can sometimes accommodate patients sooner if they are a fit for one of our team's clinical trials. The information you provide on the following intake form will help us determine wither one of our studies might be a fit for you.

If you are seeking care for yourself or your child that is more urgent and cannot wait a week to hear back from us, we are most likely not the appropriate referral for treatment. If you need urgent psychiatric care, we advise you to contact your nearest emergency room or to talk to your PCP about referral to a clinician who might be able to evaluate them sooner.
