Welcome to the intake form for the Eating Disorders Clinical & Research Program at Massachusetts General Hospital (MGH). This form is for prospective patients (and parents/guardians of minors) only. If you are a provider, please refer your patient to fill out this form on their behalf.
We are an outpatient eating disorders program located in Boston, MA. We have several clinical psychologists and one psychiatrist on staff. We are able to provide time-limited evidence-based treatment for eating disorders, typically cognitive-behavioral therapy for adults and family-based therapy for children and adolescents.
Please note that, in order to coordinate medical and mental health care, we are only able to accept patients with a pediatrician, primary care provider, or adolescent medicine physician at a Mass General Brigham member or affiliate hospital.
Other types of providers (e.g., GI providers, neurologists, psychiatrists, dietitians) are not able to meet this requirement. Further, patients must be in the state of Massachusetts for all visits with our clinic, as our providers are licensed in Massachusetts.
Providers who meet the Adolescent Medicine requirement include: Drs. Mark Goldstien, Katie Brigham, Nupur Gupta, Karen Sadler, Jennifer Rosenblum, Mariette Murphy, and Diana Lemly.

If you are seeking care for yourself or your child that is more urgent and cannot wait a week to hear back from us, we are most likely not the appropriate referral for treatment. If you need urgent psychiatric care, we advise you to contact your nearest emergency room or to talk to your PCP about referral to a clinician who might be able to evaluate them sooner.
