REDCap Initial Screening Questionnaire: Validation of Real-Time Circadian Phase Estimates

Thank you for your interest in our research study. The purpose of the study is to learn more about how various markers in blood, urine, and saliva vary throughout the day and in relation to each other. We want to use this information to develop a method to measure your internal clock time (i.e., your circadian rhythm).
If you decide to take part in this study, we would ask you to collect and measure markers from your blood, saliva, and urine several times per day. We would ask you to do this for about 24 hours at home and then come stay in our lab overnight for about 24 hours. When you stay in our lab, we will also place an IV and draw some blood samples via this IV.
In this survey, we will ask you questions about your health and medical history to find out if you might qualify for this study. Some of the questions may make you feel uncomfortable. You can stop taking the survey at any time. If you do not answer all the survey questions, we may not be able to determine if you are eligible for the study.
Your answers to this survey will be stored in our Division of Sleep and Circadian Disorders database until the study is completed. The risk of allowing us to record your name with your answers is a loss of confidentiality. We will take reasonable steps to protect the confidentiality of your information.
