Early intervention services help many families and children in Massachusetts! Unfortunately, we know that not all families and children who need EI services get them.

We want to learn about your family’s experiences when you connected with EI services for the first time. What made it harder or easier for you to get in touch with early intervention, set up an evaluation for your child, and start services? Your answers to this survey can help us make the process of accessing EI easier so other families can benefit from EI.

This survey is for all families whose child has received EI services, either now or in the past!

The survey may take approximately 10-15 minutes of your time. Your answers are completely confidential and anonymous. Your personal information, including your name and email, is not collected as part of this survey.

Throughout this survey, we will refer to early intervention services as “EI.”

This survey is part of a program improvement project between the MGH Institute of Health Professions and a community partner.
