We appreciate the effort in filling out the Massachusetts General Hospital (MGH) Center for Innovation in Digital HealthCare (CIDH) Evaluation Form. 

The questionnaire contains 5 sections and should take ~15 minutes to complete. Please fill out all the required questions to the best of your ability, and if you are unsure of a certain question, type "Not Applicable" to proceed forward. Key evaluation elements include, but are not limited to: 

1. Detailed unique value proposition of your solution(s) that address the needs of a health system, providers, and/or patients 
2. Demonstrated readiness of the solution(s) and viable plan for scalable deployment at an academic medical center or health system
3. Capability and financial resources to execute a sponsored research or co-development project at an academic medical center or health system
4. Identification of potential clinical champions or departments at MGH/Mass General Brigham (MGB) that could adopt and deploy your solution(s)
